Action Cycle: Branding

On June 6, 2012, the IPLAN Committee met to celebrate the completion of the development of the community health improvement plan and to discuss the next steps necessary to implement the plan.  Prior to June's meeting, the Branding Subcommittee began development of a brand.  After engaging in brainstorming activities and discussion, the Subcommittee recommended that the IPLAN Committee should also be engaged in brainstorming activities for additional ideas. 

Thus, the IPLAN Committee participated in brainstorming activities to assist in brand development.  The results of the creative process were compiled into a survey and Committee members were asked to cast their votes for the new name, tag line, and logo.  The brand is still under developemnt but the results will announced within the next month.

Subcommittee Presentations

On May 2, 2012, the IPLAN Committee met to share the objectives and interventions developed by each Subcommittee to address the health priorities.  The purpose of this meeting was to identify cross-cutting issues and possible opportunities for collaboration.  Attached below are copies of each Subcommittee's strategic issue worksheet that details the proposed health improvement plans.

Subcommittee Breakout Sessions

On April 4, 2012, the IPLAN Committee held a meeting specifically designated for Subcommittee breakout sessions.  The purpose of this meeting was to provide Subcommittees an additional opportunity to convene to develop their health improvement plans.  We also introduced the concept of creating a "brand" for Clay County's IPLAN as part of our marketing and promotional efforts.  The Subcommittees are making good progress and will present their health improvement intiatives at our next meeting.

3 Top Health Priorities for Clay County

On February 29, 2012, the IPLAN Committee met to select the top 3 health priorities in Clay County and to complete the Visioning process.  January's Committee meeting marked the completion of the community health assessment portion of the IPLAN process.  Upon completion of the assessment phase, the IPLAN Committee now had the information needed to select the top health priorities in Clay County.  Prior to February's meeting, the Committee members were asked to cast their votes for the top 5 health priorities utilizing the results of the assessments to inform their decision.  The votes were compiled and discussed during the Committee meeting on February 29, 2012.  After much debate and deliberation, the IPLAN Committee selected the top 3 health priorities in Clay County.  The IPLAN Committee then formed subcommittees for each health priority and selected a group leader and recorder.  Each subcommittee will be responsible for developing a health improvement plan to address its respective health priority.

Obesity / Healthy Living
Cancer
Access to Healthcare


The IPLAN Committee also created the vision statement, which marks the completion of the Visioning process. 

The selection of the top 3 health priorities and completion of the Visioning process marks the end of the community health assessment phase of the IPLAN process.  The IPLAN Committee now moving into the development of the community health improvement plan.

Survey Results

On January 25, 2012, Committee members met to conduct the Community Themes and Strengths Assessment as well as the Forces of Change Assessment.  The purpose of this meeting was to identify the assets within the community that could be utilized to drive the community health improvement plan (CHIP).  The Committee also identified possible barriers to successfully implementing the CHIP.
