Visioning

The Community Health Committee met on Wednesday, November 9, 2011 for the Visioning session.  Laurie Call, Director, Center for Community Capacity Development at the Illinois Public Health Institute, facilitated the Visioning session and led the Committee through various brainstorming activities.  We identified values and formulated these values into a mission statement and guiding principles that will direct the Committee throughout the IPLAN process.

Introduction to IPLAN

On September 28, 2011, a group of local stakeholders met at the Clay County Health Department to form the Community Health Committee.  Members of the Committee include concerned local residents that represent a variety of sectors such as health department, hospital, local government, industry, insurance, faith-based, youth, and other civic organizations.  The Committee will voice the community’s opinions and provide guidance as we conduct the community health assessment (CHA) and community health improvement plan (CHIP) in accordance with IPLAN.

During this meeting, Committee members were introduced to the requirements of IPLAN and reviewed the results from IPLAN 2007-2012.

Welcome

Welcome to the Clay County Health Department’s blog on IPLAN 2012-2017.  This website was created as a way for Clay County residents to learn more about the IPLAN process throughout the year.  Our goal is to create a community-owned health improvement plan that will make Clay County a healthier place to live, learn, and work.   
In an effort to gather community input, we invite Clay County residents to take our Community Health Survey.  Your responses will inform us of what health issues residents feel are important.  Remember - your opinion matters!
