IPLAN - Illinois Plan for the Local Assessment of Needs


The Clay County Health Department (CCHD) is underway with a special project that aims to improve the health of our county’s residents.  IPLAN, or the Illinois Project for the Local Assessment of Needs, requires that health departments throughout the state conduct a community health assessment and develop a community health improvement plan every 5 years.  The last IPLAN was completed in 2007.  Over the course of the year, CCHD, in collaboration with its public health partners, will identify the county’s most pressing health issues and gather information on the quality of life.   The results of this assessment will be used to create a community health improvement plan in which strategies and interventions are devised to improve the health and lives of Clay County residents.
